The game has changed. Its not about denials anymore its about never being asked. If you dont contract with anywhere near enough PCPs, have no urgent cares, with the ER being the only available PCP, and ER docs refusing to do PCP work, you no longer have to deny treatments becaue there is no MD to request treatments. Hope that was clear.
UK & Canada - you need to fight like hell for what you have or this is your fate.

Question: what value has he personally added to justify that?
It looks like it's simply that his power within the company means that he can raid the collective till and put himself at the front of the remuneration queue. That's rent, not work.
